Before filling out the application please read the following instructions carefully.
 
There are three parts to this application and all three parts must be submitted
in order to be considered eligible for potential funding.
 
This Artist Submission Form and payment may be delivered to The Foundation electronically or by mail.  If you choose to mail in your application, photo, artist audition (CD or cassette) and check, just print out this page, legibly fill in all of the appropriate information
and address it to:

If you choose to submit your Artist Submission Form directly to us from this page
please remember that the complete application comes in THREE parts.
Fill out Part One and send it to us by selecting the SUBMIT button at the end of Part One.
 
Part Two will be an email with your photo and mp3 addressed directly to the
Shine On Foundation Executive Director Susie Marshall at:

Part Three is for your payment submission.
  
The application process is not complete until all three parts have been submitted.
Once all three parts have been received at our offices you will receive an email within
